Admissions Registered Nurse

Full-Time Position

Position Summary 

Act as a liaison between the facilities and the hospice and palliative care team. Initiates and evaluates patient admissions for the appropriate hospice and palliative care services in accordance with regulation admission and eligibility requirements. Responsible for the following programs: Hospice, Supportive Care, Kupu Care, and Transitions


Duties & Responsibilities

  • —Initiates the admission process, by educating and informing patient and/or family of Hawaii Care Choices services.

  • —Evaluates pre-admissions for the appropriate hospice and palliative care services in accordance with regulation, admission, and eligibility guidelines.

  • —Consults the Director of Clinical Services (DCS) and the Medical Director as necessary on difficult cases

  • —Acts as advisor for pain and symptom management, and attends patient family conferences at facilities as necessary

  • —Sustains a scheduled presence at Hawaii Medical Center (HMC) and skilled nursing facilities by autonomously organizing her/his  schedule

  • —Effectively collaborates with and communicates detailed admission reports to other Clinical disciplines including Physician and/or APRN, Social Work, and Spiritual Care.

Skill & Knowledge Requirements

  • —Critical thinking  skills and judgment in the admission process for patients and their families

  • —Ability to provide unsupervised health care services in private residences, along with handling and administering controlled and prescribed medications.

  • —Ability to identify and provide for psychosocial needs of the patient and care givers. 

  • —Ability to coordinate care with the interdisciplinary team. 

  • —Requires excellent critical thinking and assessment skills.

  • —Interest and desire to give palliative and hospice care with a special sensitivity to the needs of the dying.

Education & Experience Requirements


  • —Graduate of a registered nursing curriculum; BSN degree or higher.

  • —Possession and maintenance of a valid Hawai’i State nursing license.

  • —Valid CPR certification.

  • —Minimum of two (2) years of experience in a health care organization with similar patient care situations
