Community Bereavement Counselor

Position Summary

Provides and/or supervises supportive psychosocial and bereavement services for the surviving families and the community-at-large via groups, one-on-one sessions, and as consultant to the IDT.  Implements the bereavement program within the letter and spirit of

hospice under Medicare, including various grief and bereavement groups.  Supports the hospice philosophy and team approach to provide, with other members of the team, holistic care to the patient and family.

Duties & Responsibilities

  • Works with Leadership and Interdisciplinary Team on bereavement needs of patients/ community members per organization processes.

  • Assist the Clinical Leadership in managing the bereavement programs utilizing professional staff and volunteers as appropriate to the service or event.

  • Assesses psychological, social, and environmental needs of the family.  Care may be provided in the family's residence or the hospice office.

  • Provides counseling and social services, including but not limited to counseling, grief processing, consultation, education, resource and referral services based on the bereavement assessment and plan of care.

  • Participates in Interdisciplinary Team meetings as needed, contributing to the development and continuous updating of the bereavement plan of care.

  • Documents all counseling and social interactions in accordance with Hawaii Care Choices' regulations.  Maintains documentation of time sheets, mileage, and other records as required.

  • Provides bereavement consultation and support in response to community needs as assigned.

  • Assist with educational programs for patients/families, staff and the community on loss, grief, and coping with bereavement.

  • Upholds and maintains the organization's cost containment efforts, without jeopardizing the quality of hospice care.  

  • Observes and promotes sound safety practices. 

  • Follows all policies and procedures of Hawaii Care Choices.

  • Provides education and training to the patients, care givers, volunteers, co-workers and the community at large as deemed necessary and as requested.

  • Participates in meetings, committees, program development, and special projects as requested.

  • Identify resources in the community for those grieving and expectant loss.

  • Assist hospice grieving families with community resources to ease the burden of loss.

 

Skills & Knowledge Requirements

  • Ability to exercise independent sound judgment in planning and providing quality psychosocial care.

  • Possession of critical assessment and thinking skills.

  • Ability to provide unsupervised psychosocial services and support in private residences.

  • Self-motivation and ability to work independently as well as in cooperation with the interdisciplinary team.

  • Interest and desire to give hospice care with a special sensitivity to the needs of the dying.

Education & Experience Requirements

  • Graduate of a school accredited by the Council of Social Work Education, with a minimum of a Masters degree in social work, or licensed as a Marriage and Family Therapist (LMFT).

  • —Prefer:   Hospice experience.

Hours:

      

  • —Primarily Monday through Friday 8:00am – 4:30pm. Possibility of evening, weekend and holiday work.

Pay: $66,500 - $71,500/ annually

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
