Location: Palliative Care Program

Position Summary

Works closely under the supervision of the Director of Clinical Services. Holistically assess, plan, implement, and evaluate programs of evidence-based, highly specialized advanced palliative care. Provides expert advice, guidance, and support to patients, caregivers/relatives, clinical practitioners, and peers. Provides excellent interpersonal and communication skills. Works collaboratively and cooperatively with clinical colleagues to develop services and quality of care delivery. Participates in the delivery of specialist education and training. Undertake research and knowledge.

Duties & Responsibilities

  • Use excellent communication skills to support those with an illness and their family/caregivers.

  • Establish and maintain effective communication with professionals across health and social care services.

  • Influence, negotiate, challenge, and empower all health and social care professionals to develop and deliver the highest-quality individualized palliative care.

  • Lead and collaborate with other professionals and agencies to ensure patients' specialist palliative care needs are met, including ongoing needs incorporating psychological problems, spiritual issues, social and financial needs, and discharge arrangements.

  • Work autonomously across hospital and community boundaries.

  • Support specialist palliative care multidisciplinary meetings and develop planning.

  • Evaluate service delivery, identify areas for improvement, and initiate change in consultation with senior management.

  • Delegate admin tasks to the admin team appropriately to ensure best time management.

  • Keep up to date with developments in specialist palliative care to ensure advanced clinical practice and education programs represent current best practices.

  • Design productivity strategies for financial acumen and streamline workflow/ procedures to maximize staff utilization and productivity.

  • Develops strategies, expectations, and outreach to launch palliative care in multiple settings.

  • Design growth impact in service amenities.

Skill & Knowledge Requirements

  • Strong leadership ability in a fast-paced environment.

  • Excellent interpersonal, communication, and critical thinking skills.

  • Ability to work with a diverse team while holding the team accountable for productivity goals.

  • Ability to discern sensitive issues, respond appropriately, and react quickly.

  • Knowledge and understanding of the hospice and palliative care philosophy and hospice and palliative care criteria for admission and the admission process.

Educational and Experience Requirements

  • MBA with a focus on Health Care Management.

  • Minimum of (2) years experience in the Health Care Industry with a focus on project management and implementing/ monitoring quality improvement plans.

  • Hospice and/or palliative experience care preferred.

Hours

  • Generally, Monday through Friday during regular business hours.

  • Holidays and weekends may be required as needed.

Pay

$93,600 - $112,000 / annually

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.​
