Bereavement Counselor
Per Diem Position
Position Summary
Provides and/or supervises supportive psychosocial and bereavement services for the surviving families and the community-at-large via groups, one-on-one sessions, and as a consultant to the IDT. Implements the bereavement program within the letter and spirit of hospice under Medicare, including various grief and bereavement groups. Supports the hospice philosophy and team approach to provide, with other members of the team, holistic care to the patient and family.
Duties & Responsibilities

Care for bereaved by providing telephonic, in person counseling, and engage in the various support groups HiCARE offers throughout the year.

Leadership in managing the bereavement programs utilizing professional staff and volunteers as appropriate to the service or event.

Assesses psychological, social and environmental needs of the family. Care may be provided in the family's residence or the hospice office.

Provides counseling and social services, including but not limited to counseling, grief processing, consultation, education, resource and referral services based on the bereavement assessment and plan of care.

Participates in Interdisciplinary meetings as needed, contributing to the development and continuous updating of the bereavement care plan.

Documents all counseling and social interactions in accordance with Hawaii Care Choices regulations.

Assist with educational programs for patients/families, staff and the community in loss, grief and coping with bereavement.
Skills & Knowledge Requirements

Ability to exercise independent sound judgment in planning and providing quality psychosocial care.

Possession of critical assessment and thinking skills.

Ability to provide unsupervised psychosocial services and support in private residences.

Self-motivation and ability to work independently as well as in cooperation with the interdisciplinary team.

Interest and desire to give hospice care with a special sensitivity to the needs of the dying.
Education & Experience Requirements

Minimum: A Masters degree in Social Work or an equivalent degree from an accredited college or university. Minimum of two (2) years experience in a specialty area similar to the hospice program of care. Demonstrated clinical experience in the areas of life-threatening and chronic illness, grief and loss, individual, family and group therapy. Prefer: Hospice experience.
Hours:

Primarily Monday through Friday 8:00am – 4:30pm. Possibility of evening, weekend and holiday work.
