Certified Nurse Aide - Homecare

Position Summary

  • This position, under the direction of a Registered Nurse, is responsible for providing a variety of direct patient care and/or homemaker support services and tasks in accordance with the guidelines of the Interdisciplinary Team Plan of Care and established policies and procedures. These services are provided throughout the Hospice of Hilo service area of South Point Road to Laupahoehoe Point. 

Duties & Responsibilities

  • —Follows assigned duties and instructions as delegated by the Hospice RN and Clinical Team Leader.

  • —Completes visits as scheduled and assigned by the CNA Scheduler.

  • —Employs appropriate and safe techniques in performing personal hygiene, grooming tasks, assistance in activities of daily living, and homemaker services to hospice patients in compliance with federal, state, and hospice regulations.

  • —Documents consistently all care, services and observations in accordance with hospice protocol and keeps RN informed of changes in patient's condition.  Maintains documentation of time sheets, mileage and other records as required, in a timely manner.

  • —Demonstrates good communication skills, including ability to read, write, and verbally report clinical information to supervising RN, Clinical Team Leader and other team members.

  • —Upholds and maintains the organization's cost containment efforts without jeopardizing the quality of hospice care.

  • —Observes and promotes sound safety practices; follows all policies and procedures of Hospice of Hilo.

Skills & Knowledge Requirements

  • —Demonstrate knowledge of basic nursing skills (i.e., taking vital signs, measuring output, recognition of abnormal signs and symptoms, etc.); patient rights; standard precautions; infection control; body mechanics; purpose and benefits of activities of daily living; personal hygiene care; basic medical and nursing terminology.

  • —Ability to independently provide direct personal care services to patients in their homes; ability to work with patients and families coping with a terminal illness.

  • —Ability to exercise judgment in provision of care and communication.

  • —Ability to adapt to different lifestyles, cultures and living arrangements.

  • —Interest and desire to give hospice care with a special sensitivity to the needs of the dying.

  • —Maintenance of adequate educational and experiential level to meet the needs of the patients and care givers and regulations established by state and federal laws.

  • —Possession and maintenance of a valid State of Hawai’i Nurse Aide Certificate certification.

Educational and Experience Requirements

  • —State Certification from a State of Hawai’i Certified Nurse Aide Training Program.

  • —At least two years CNA experience in home health or hospice care in the community.

Hours

  • —Must be flexible.

  • —Primarily Monday to Friday, 8:00 a.m. to 4:30 p.m., with possibility of evening, weekend and holiday work. 

 

Pay

  • $18.00-$20.00/hour

  • This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
