Position Summary
Serve as the primary sales and relationship liaison between Hawai‘i Care Choices and licensed Care Homes across the island to drive hospice and palliative care census growth.
Develop, manage, and grow referral partnerships that align with the organization’s mission and strategic growth goals.
Duties & Responsibilities

Build and maintain active referral relationships with Care Home operators, administrators, nurses, and care staff.

Responsible for establishing strong & meaningful relationships with all stakeholders.

Promote and market Hawaii Care Choices through sales calls, in-service presentations, grand rounds, industry conferences/fairs, etc.

Create, organize, and execute educational sales presentations.

Develop and execute a Care Home–specific outreach strategy that results in consistent, measurable referral growth.

Conduct regular in-person visits to assigned Care Homes to maintain visibility, trust, and education.

Educate Care Home partners on: Hospice eligibility and timing, Palliative care appropriateness, Referral processes and expectations.

Identify new Care Home opportunities and expand market penetration across the island.

Collaborate with Intake, Clinical Leadership, and Operations to ensure: Smooth referral-to-admission transitions, Timely follow-up on leads, High partner satisfaction.

Represent Hawai‘i Care Choices professionally at Care Home meetings, trainings, and community events.

Track all outreach activity, referrals, and outcomes in the designated CRM or tracking system.

Serve as a Sales Representative for other clinics as determined by Supervisor.

Excellent written and verbal communication skills as well as the ability to communicate effectively with all levels of individuals.

Basic computer knowledge of MS applications such as Word, Excel, Outlook.
Education & Experience
-
Bachelor’s Degree in Marketing, Communications, or Healthcare Administration is preferred.

Minimum of one (1) year of experience in direct sales experience is preferred.

Healthcare environment experience is preferred.

Frequent use of private vehicles.
-
Occasional use of computer, fax machine, hole puncher, paper shredder, paper cutter, copier and general office equipment.
Work Hours

Exempt position: requires flexibility and may be required to work after-hours, on weekends, and holidays as needed.
Pay $68,000 - $69,000/ annually
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
