Position Summary

Provides services utilizing principles of palliative care and the hospice philosophy. Provides direct patient/family counseling and referrals to appropriate community resources as needed. Assists the patient and family in developing and enhancing optimal coping strategies. Supports the hospice philosophy and team approach to provide, with other members of the team, holistic care to the patient and family
Duties & Responsibilities

Provides social work services, including but not limited to: psychosocial assessment, counseling, consultation, education, resource and referral services, advocacy, and discharge planning, based on the social work assessment and plan of care, with the goal to empower the patient and family through education and support.

Participates in developing the interdisciplinary care plan both as a consultant and as a provider of direct services. Participates in the weekly interdisciplinary team conferences.

Responsible for the initial post death bereavement assessment and coordinates after-death bereavement plan of care .
Skill & Knowledge Requirements

Ability to exercise independent sound judgment in planning and providing quality psychosocial care.

Possession of critical assessment and thinking skills.

Interest and desire to give hospice care with a special sensitivity to the needs of the dying.

Maintains adequate education and experience level to meet the needs of the patients, care givers, and regulations established by State and Federal laws.
Educational and Experience Requirements

Graduate of a school accredited by the Council of Social Work Education, with a minimum of a Masters degree in social work.

LSW license required.

Hospice experience and LCSW preferred.
Hours

Exempt position. Scheduled, staggered weekdays and weekends, normally 40 hours/week with occasional evening and holiday hours.
