Position Summary
Under supervision, and in collaboration with the Medical Director or designee, the Nurse Practitioner provides hospice care services to patients and families in a variety of settings. Responsible for performing comprehensive medical and psychosocial evaluations. Performs clinical care of potential and current patients and families faced with life threatening or progressive, life-limiting health conditions. Includes administrative oversight and management of outpatient clinic.
Duties & Responsibilities

Performs patient evaluations in outpatient and inpatient settings.

As credentialed and privileged to perform, physical examinations, histories, chart documentation, patient orders, radiological and laboratory interpretation, discharge orders, patient & family instruction and education, and participation in case management.

Provides services related to end-of-life care, assesses patient needs, and anticipates service requirements prior to discharge or change of place of service.

Coordinates with the Director of Clinical Services, Clinical Team Leader, clinicians and applicable ancillary staff regarding care services.

Provides services related to palliative, and end of life care, assesses patient needs, and anticipates service requirements prior to discharge or change of place of service.

Prescribes medications including controlled substances, to the extent delegated and licensed.

Orders treatments and durable medical equipment as indicated.

Performs other diagnostic and therapeutic measures as indicated.

Educate clinical staff, patients, and families on applicable disease management and prevention strategies.

Maintains confidentiality regarding patients, families, staff, and the organization.

Assists with the coordination of admissions and discharges as needed.

Works collaboratively with physicians and other health care providers to optimize patient care.

Consults with the Medical Director or designees as needed.

Informs primary physician of services.

Provides written and/or verbal reports to primary care physicians and specialist for each consultative service.

Prepares and maintains accurate patient records, charts and documents to support sound palliative care practice and appropriate reimbursement for services provided in a timely fashion.

Assists in the development of clinical documentation protocols.

Initiates reimbursement for services rendered and evaluations. ·Makes visits to palliative care and hospice patients as delegated by the Medical Director or designee.

Reports significant changes in the patient’s condition as required by agency policy; both verbally and in writing, to the physician and other appropriate agency personnel.

Consults with the clinical supervisor as necessary.

Maintains regulatory compliance in all areas of position responsibilities.

Maintains coordination with other disciplines as needed for patient care.

Provides appropriate resources in accordance with agency policies and procedures.

Appropriately communicates in solving problems as they arise.

Is responsible for his/her continuing education, e.g. professional journals, attending seminars, workshops, assisting in presentation of in-service education and community education.

Collects and assesses patient care data, develops and implements a plan of care, manages and evaluates outcome of care.

Collects, assesses and participates in gathering and monitoring quality of care metrics.

Participates as an active member of the Interdisciplinary Team (IDT).
Skill & Knowledge Requirements

Full understanding and compliance with State of Hawaii and federal laws and regulations on nursing and nurse practitioner scope of work; responsibilities, professional principles, procedures and techniques of nurse practitioners as applicable within specific licensure.

Advanced knowledge of symptoms and normal course of acute and chronic illnesses, and their interventions/treatments.

Ability to chart on laptop in preparing concise, legible, timely clinical notes, according to agency guidelines.

Ability to work independently and apply sound medical judgment and hospice protocols.
Education & Experience

Licensed State of Hawai’i Nurse Practitioner (NP) in a specialty area that permits NP to diagnose, treat and prescribe to adult patients ages 18 and older (APRN RX).

Valid State of Hawai’i RN license and a Masters in Nursing (MSN) from an accredited Nurse Practitioner Program required.

Hold and maintain unrestricted license for full prescribing authority based on state and federal requirements .

Credentialed and privileged with health plans and facilities as required to perform duties and meet agency needs.

Valid CPR certification.

Primarily Monday through Friday, 8:00 a.m. to 5:00 p.m., with occasional evening and weekend work as needed to accomplish the goals and objectives of the position. May be required to participate in on-call coverage with Hospice Physicians.
Pay $120,000 - $125,000/ annually
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
