Per Diem 

Position Summary 

Provides skilled nursing to hospice patients and their families outside of normal office hours.  Is available during these times for problems experienced by hospice patients and their care givers.  Makes home visits when necessary for emergencies and deaths. Utilizes principles of palliative care and hospice philosophy to provide holistic care. May be asked to provide RN coverage during regular business hours as needed.

Duties & Responsibilities

  • —Responsible for obtaining current patient information and physician orders by collaborating with the Hospice Registered Nurse, and designated Supervisor(s).

  • —Is available by pager or phone at all times while on duty.  Returns calls within 15 minutes of receiving a page or phone contact.

  • —Provides nursing care to patients and support to care givers, following the plan of care and palliative guidelines to the extent possible.

  • —When indicated, gives report to the attending physician and/or obtains new medication orders; assesses the need for skilled nursing by phone.

  • —Makes home visits as needed when: 

    • family distressed and cannot be calmed over phone

    • skilled nursing interventions required.

    •  patient is newly admitted to hospice service and requires additional education, support.

    • drastic change in the patient’s physical condition occurs.

    • patient has expired.

  • —Be available for scheduled calls, daily visits to general inpatient and respite hospitalized patients or provide continuous care, as assigned.

  • —Reports any administratively complicated situations to the Clinical Manager, Director of Clinical Services or designee.

  • —Reports any clinically complicated situations to the Medical Director.

  • —Per Diem RN will be available (as scheduled) to provide phone coverage, home visits, scheduled visits, and death visits on an as needed basis.

  • —Documents all calls and home visits in accordance with Hawaii Care Choices protocol.  Maintains documentation of time sheets, mileage and other records as required.

  • —Reports to Hospice Registered Nurse, Clinical Manager or the Director of Clinical Services any patient concerns to ensure appropriate continuity of care.

  • —Performs case manager duties as needed and may on occasion process referrals, intakes and admissions.

Skill & Knowledge Requirements

  • —Ability to exercise independent sound judgment in planning and providing quality hospice nursing care. 

  • —Ability to provide unsupervised health care services in private residences, as well as to handle and administer controlled and prescribed medications. 

  • —Ability to identify and provide for psychosocial needs of the patient and care givers. 

  • —Duties require excellent critical thinking and assessment skills.

  • —Adequate educational and experiential level to meet the needs of the patients and care givers and regulations established by State and Federal law.

  • —Possession and maintenance of a valid Hawai’i State nursing license.

  • —Possession of current CPR Certification.

Educational and Experience Requirements

  • —Graduate of a RN Nursing Curriculum; BSN preferred. 

  • —Minimum two (2) years of experience in a health care organization with similar patient care situations.

Hours

  • —Scheduled coverage will be scheduled in advance, and is dependent on the agency's needs.

  • —On-Call hours begin at 4:30 p.m. to 8:00 a.m. weekdays; weekend call is 4:30 p.m. Friday through 8:00 a.m. Monday; and holidays. Please see attached signed “On-Call Scheduling Commitment”.

Pay

$38.00 - $42.00 per hour

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
